HEALTH and social care partners in Dorset are working together to improve the lives of frail older people and people with multiple long-term conditions.

They are calling on residents to share their views to help develop a draft description of how services can work together in the future.

The work is part of the Better Together programme for the county and a recent event attracted more than 100 people from a range of health and social care organisations.

Dr Forbes Watson, chairman of NHS Dorset Clinical Commis-sioning Group, said: “We gathered views from a large number of people representing a wide range of organisations.

“We have used these to create the first draft of proposals, on which we are asking for views.

“Input from local people is vital in everything we do and we are encouraging people to visit our website and have their say”.

Dorset County Council leader Spencer Flower said: “Many excellent projects are being developed by working together. Staff from all agencies can readily identify which people are most at risk, such as those going into hospital.

“It will be possible to put together combined packages of care coupled with support and lifestyle advice to keep them healthier and independent for much longer.”
